All forms necessary to compete in representative teams chosen for City District 

Personal Details, Parental Consent and Medical Details forms need to be completed by parents before the trial. Availability for selection must be signed by school principal (or nominee) before trial.

Code of Conduct Forms need to be read as their conditions are binding upon those invited to be part of District teams
